PLANNNED MAINTENANCE SYSTEM (PMS)

1.0 PURPOSE

The main purpose objectives of this procedure are as follows:-
a. To maintain the vessels’ machinery and equipment in good running condition so that breakdowns and defects are minimized or avoided during operation.
b. To provide a comprehensive and up-to-date monitoring of all maintenance works done or to be done.
c. To assist in prompt identification and trouble-shooting of potential breakdowns or defects.
d. To provide feedback to shore staff on condition and performance of selected machinery and equipment.
e. To maintain comprehensive records of maintenance and condition of vessels, machinery and equipments.

2.0 SCOPE
The planned maintenance system (PMS) shall apply to all vessel managed or operated by
Azimuth Ship Management, as listed in the company QSMS.

3.0 DEFINITION

3.1 Deck Servicing Plan (Form DSP)
This Servicing Plan is used to cover the whole-year planned maintenance interval of all
Deck equipment, fittings and structures. It defines the maintenance interval and allows for
the maintenance completion dates to be filled in. Refer to the specimen attached at the
end this section.

3.2 Engine Room servicing Plan (Form: ESP)
This Plan is used to cover the whole-year planned maintenance interval of all Engine
Room machinery. It defines the maintenance interval and allows for the maintenance
completion dates to be filled in. Refer to the specimen attached at the end of this section.

3.3 Deferred Maintenance Form (Form: DMF)
This form is used for reporting any planned maintenance work not carried out within the
maintenance interval specified in the Servicing Plan. The reason for not carrying out the
planned maintenance work must be included. Should all the planned maintenance works be
completed according to the Servicing Plan, then this form need not be submitted for the
particular month. The same form is to be used for both the Deck and Engine Room
departments. Refer to the specimen attached at the end of this section.

3.4 Monthly Report (Form: MR)
This Form is used to summaries’ the planned maintenance work done for the month and
the completion dates. The data are extracted from the Deck and engine Room Servicing
Plans. The same form is to be used for both the Deck and Engine Room departments.
Refer to the specimen attached at the end of this section.

3.5 Hull/Deck Condition Report (Form: HCR)
This form shall be used by the Deck department to report any breakdowns, defects or
deficiencies noted during the month, on the hull/deck structure, equipment, machinery and
gear. It shall give a brief description of the deficiencies or defects; possible known causes;
and any repairs done. Refer to the specimen attached at the end of this section.

3.6 Machinery Condition Report (Form: MCR)
This Form shall be used by the Engine Room department to report any breakdowns,
defects or deficiencies noted during the month, on any Engine Room machinery and
equipment. It shall give a brief description of the deficiencies or defects; possible known
causes; and any repairs done. Refer to the specimen attached at the end of this section.

RESPONSIBILITIES OF OFFICERS-

Technical Superintendent (TS) – The TS is to ensure that:
a) The Chief Engineer carries out the Engine Room Planned maintenance according
to the Engine Room Servicing Plan.
b) The Chief Engineer submits the office copies of all the Engine Room departments
PMS forms, except for the Engine Room Servicing Plan on time.
c) His authorized assistant fill in the office copy of the Engine Room Servicing Plan
according to the Monthly Report received from the vessel.
d) Review, investigate, and ensure that all the engine Room department deferred
maintenance reported in the Deferred Maintenance Form are carried out at the
earliest opportunity.
e) Review, investigate, provide all necessary support and ensure that all the engine
Room department breakdowns and deficiencies reported in the Machinery
Condition Report are rectified in a timely manner.
f) File and maintain office copies of all the Engine Room department PMS forms.

The TS is to ensure that:
a) The Master carry out the Deck planned maintenance according to the Deck
Servicing Plan.
b) The Master submit the office copies of all the Deck department PMS forms, except
for the Deck Servicing Plan on time.
c) His authorized assistant fill in the office copy of the Deck Servicing Plan according
to the Deck Department Monthly Report received from the vessel.
d) Review, investigate and ensure Deck department deferred maintenance were
reported in the Deferred Maintenance Form are carried out at the earliest
opportunity.
e) Review, investigate provide all necessary support, and ensure that all Deck
department breakdowns and deficiencies reported in the Hull/Deck Condition
Report are rectified in a timely manner.
f) File and maintain office copies of all the Deck department PMS forms.

MASTER’S RESPONSIBILITIES-

The Master‘s responsibilities include:
a) Carrying out the Deck department planned maintenance according to the Deck
Servicing Plan
b) Submitting copies of all the Engine Room department PMS forms to the office at
the end of each month, except for the Engine Room Servicing Plan which is to be
retained onboard.

c) Reports any Engine Room department deferred maintenance in the Deferred
Maintenance Form, and ensure that all deferred maintenance are carried out at
the earliest opportunity.
d) Report all Engine Room breakdowns and deficiencies in the Machinery Condition
Report, carried out permanents or temporary repairs in a timely manner, and
request OHM foe any required assistance.
e) File and maintain shipboard copies of all the Engine Room PMS forms.
f) Ensuring that submission of PMS forms as described in 3.7 are not interrupted
even when his vessel is undergoing dry docking, Special Surveys major retrofitting
of equipment, owner’s maintenance, charterer’s requirement,etc

Chief Engineer Responsibilities-

The Chief Engineer’s responsibilities include:
a) Carrying out the Engine Room department planned maintenance according to the
Engine Room Servicing Plan.
b) Submitting copies of all the engine Room department PMS forms to the office at
the end of each month, except for the Engine Room Servicing Plan which is to be
retained on board.
c) Reports any Engine Room department deferred maintenance in the Deferred
Maintenance Form, and ensure that all deferred maintenance are carried out at
the earliest opportunity.
d) Report all engine Room department’s breakdowns and deficiencies in the
Machinery Condition Report, carry out permanent or temporary repairs in a timely
manner, and request OHM for any required assistance.
e) File and maintain shipboard copies of all the Engine Room PMS forms.
f) Ensuring that submission of PMS forms as described in 3.7 are not interrupted
even when his vessel is undergoing drydocking, Special Surveys, Major retrofitting
of equipment, owner’s maintenance, charterer’s requirements, etc.

PROCEDURE

Form Issuance
a) At the end of each year, the OHM is to issue full sets of all PMS forms to the
vessel’s Deck and Engine Room departments. He is to ensure that these are
enough for one year usage, with an appropriate amount of spare copies.
b) OHM is also to issue sufficient sets of engine Room Servicing Plans for his
updating of the office copies.
c) OHM will also affect the Deck Servicing Plans for his own usage.
d) These issuance are to be carried in accordance to QSMS 2.3.1: Controlled of
