To maintain the vessels\u2019 machinery and equipment in good running condition so that breakdowns and defects are minimized or avoided during operation.<br \/>\nb. To provide a comprehensive and up-to-date monitoring of all maintenance works done or to be done.<br \/>\nc. To assist in prompt identification and trouble-shooting of potential breakdowns or defects.<br \/>\nd. To provide feedback to shore staff on condition and performance of selected machinery and equipment.<br \/>\ne. To maintain comprehensive records of maintenance and condition of vessels, machinery and equipments.<\/p>\n<p><strong>2.0 SCOPE<\/strong><br \/>\nThe planned maintenance system (PMS) shall apply to all vessel managed or operated by<br \/>\nAzimuth Ship Management, as listed in the company QSMS.<\/p>\n<p><strong>3.0 DEFINITION<\/strong><\/p>\n<p><strong>3.1 Deck Servicing Plan (Form DSP)<\/strong><br \/>\nThis Servicing Plan is used to cover the whole-year planned maintenance interval of all<br \/>\nDeck equipment, fittings and structures. It defines the maintenance interval and allows for<br \/>\nthe maintenance completion dates to be filled in. Refer to the specimen attached at the<br \/>\nend this section.<\/p>\n<p><strong>3.2 Engine Room servicing Plan (Form: ESP)<\/strong><br \/>\nThis Plan is used to cover the whole-year planned maintenance interval of all Engine<br \/>\nRoom machinery. It defines the maintenance interval and allows for the maintenance<br \/>\ncompletion dates to be filled in. Refer to the specimen attached at the end of this section.<\/p>\n<p><strong>3.3 Deferred Maintenance Form (Form: DMF)<\/strong><br \/>\nThis form is used for reporting any planned maintenance work not carried out within the<br \/>\nmaintenance interval specified in the Servicing Plan. The reason for not carrying out the<br \/>\nplanned maintenance work must be included. Should all the planned maintenance works be<br \/>\ncompleted according to the Servicing Plan, then this form need not be submitted for the<br \/>\nparticular month. The same form is to be used for both the Deck and Engine Room<br \/>\ndepartments. Refer to the specimen attached at the end of this section.<\/p>\n<p><strong>3.4 Monthly Report (Form: MR)<\/strong><br \/>\nThis Form is used to summaries\u2019 the planned maintenance work done for the month and<br \/>\nthe completion dates. The data are extracted from the Deck and engine Room Servicing<br \/>\nPlans. The same form is to be used for both the Deck and Engine Room departments.<br \/>\nRefer to the specimen attached at the end of this section.<\/p>\n<p><strong>3.5 Hull\/Deck Condition Report (Form: HCR)<\/strong><br \/>\nThis form shall be used by the Deck department to report any breakdowns, defects or<br \/>\ndeficiencies noted during the month, on the hull\/deck structure, equipment, machinery and<br \/>\ngear. It shall give a brief description of the deficiencies or defects; possible known causes;<br \/>\nand any repairs done. Refer to the specimen attached at the end of this section.<\/p>\n<p><strong>3.6 Machinery Condition Report (Form: MCR)<\/strong><br \/>\nThis Form shall be used by the Engine Room department to report any breakdowns,<br \/>\ndefects or deficiencies noted during the month, on any Engine Room machinery and<br \/>\nequipment. It shall give a brief description of the deficiencies or defects; possible known<br \/>\ncauses; and any repairs done. Refer to the specimen attached at the end of this section.<\/p>\n<p><strong>RESPONSIBILITIES OF OFFICERS-<\/strong><\/p>\n<p><strong>Technical Superintendent (TS) &#8211; <\/strong><strong>The TS is to ensure that:<\/strong><br \/>\na) The Chief Engineer carries out the Engine Room Planned maintenance according<br \/>\nto the Engine Room Servicing Plan.<br \/>\nb) The Chief Engineer submits the office copies of all the Engine Room departments<br \/>\nPMS forms, except for the Engine Room Servicing Plan on time.<br \/>\nc) His authorized assistant fill in the office copy of the Engine Room Servicing Plan<br \/>\naccording to the Monthly Report received from the vessel.<br \/>\nd) Review, investigate, and ensure that all the engine Room department deferred<br \/>\nmaintenance reported in the Deferred Maintenance Form are carried out at the<br \/>\nearliest opportunity.<br \/>\ne) Review, investigate, provide all necessary support and ensure that all the engine<br \/>\nRoom department breakdowns and deficiencies reported in the Machinery<br \/>\nCondition Report are rectified in a timely manner.<br \/>\nf) File and maintain office copies of all the Engine Room department PMS forms.<\/p>\n<p><strong>The TS is to ensure that:<\/strong><br \/>\na) The Master carry out the Deck planned maintenance according to the Deck<br \/>\nServicing Plan.<br \/>\nb) The Master submit the office copies of all the Deck department PMS forms, except<br \/>\nfor the Deck Servicing Plan on time.<br \/>\nc) His authorized assistant fill in the office copy of the Deck Servicing Plan according<br \/>\nto the Deck Department Monthly Report received from the vessel.<br \/>\nd) Review, investigate and ensure Deck department deferred maintenance were<br \/>\nreported in the Deferred Maintenance Form are carried out at the earliest<br \/>\nopportunity.<br \/>\ne) Review, investigate provide all necessary support, and ensure that all Deck<br \/>\ndepartment breakdowns and deficiencies reported in the Hull\/Deck Condition<br \/>\nReport are rectified in a timely manner.<br \/>\nf) File and maintain office copies of all the Deck department PMS forms.<\/p>\n<p><strong>MASTER&#8217;S RESPONSIBILITIES-<\/strong><\/p>\n<p><strong>The Master\u2018s responsibilities include:<\/strong><br \/>\na) Carrying out the Deck department planned maintenance according to the Deck<br \/>\nServicing Plan<br \/>\nb) Submitting copies of all the Engine Room department PMS forms to the office at<br \/>\nthe end of each month, except for the Engine Room Servicing Plan which is to be<br \/>\nretained onboard.<\/p>\n<p>c) Reports any Engine Room department deferred maintenance in the Deferred<br \/>\nMaintenance Form, and ensure that all deferred maintenance are carried out at<br \/>\nthe earliest opportunity.<br \/>\nd) Report all Engine Room breakdowns and deficiencies in the Machinery Condition<br \/>\nReport, carried out permanents or temporary repairs in a timely manner, and<br \/>\nrequest OHM foe any required assistance.<br \/>\ne) File and maintain shipboard copies of all the Engine Room PMS forms.<br \/>\nf) Ensuring that submission of PMS forms as described in 3.7 are not interrupted<br \/>\neven when his vessel is undergoing dry docking, Special Surveys major retrofitting<br \/>\nof equipment, owner\u2019s maintenance, charterer\u2019s requirement,etc<\/p>\n<p><strong>Chief Engineer Responsibilities-<\/strong><\/p>\n<p><strong>The Chief Engineer\u2019s responsibilities include:<\/strong><br \/>\na) Carrying out the Engine Room department planned maintenance according to the<br \/>\nEngine Room Servicing Plan.<br \/>\nb) Submitting copies of all the engine Room department PMS forms to the office at<br \/>\nthe end of each month, except for the Engine Room Servicing Plan which is to be<br \/>\nretained on board.<br \/>\nc) Reports any Engine Room department deferred maintenance in the Deferred<br \/>\nMaintenance Form, and ensure that all deferred maintenance are carried out at<br \/>\nthe earliest opportunity.<br \/>\nd) Report all engine Room department\u2019s breakdowns and deficiencies in the<br \/>\nMachinery Condition Report, carry out permanent or temporary repairs in a timely<br \/>\nmanner, and request OHM for any required assistance.<br \/>\ne) File and maintain shipboard copies of all the Engine Room PMS forms.<br \/>\nf) Ensuring that submission of PMS forms as described in 3.7 are not interrupted<br \/>\neven when his vessel is undergoing drydocking, Special Surveys, Major retrofitting<br \/>\nof equipment, owner\u2019s maintenance, charterer\u2019s requirements, etc.<\/p>\n<p><strong>PROCEDURE<\/strong><\/p>\n<p><strong>Form Issuance<\/strong><br \/>\na) At the end of each year, the OHM is to issue full sets of all PMS forms to the<br \/>\nvessel\u2019s Deck and Engine Room departments. He is to ensure that these are<br \/>\nenough for one year usage, with an appropriate amount of spare copies.<br \/>\nb) OHM is also to issue sufficient sets of engine Room Servicing Plans for his<br \/>\nupdating of the office copies.<br \/>\nc) OHM will also affect the Deck Servicing Plans for his own usage.<br \/>\nd) These issuance are to be carried in accordance to QSMS 2.3.1: Controlled of<br \/>\nDocument<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>PLANNNED MAINTENANCE SYSTEM (PMS) 1.0 PURPOSE The main purpose objectives of this procedure are as follows:- a.
